Lower Back Pain Treatment - San Diego
In San Diego, back pain, especially low back pain, is a fairly common condition. Low back pain treatment begins with a complete understanding of the anatomy of spine and the lower back. The lower back carries most the weight of the torso, making it the most frequently injured area of the spine.

When you look at the anatomy of the spine there are structures that can be the source of low back pain:

Vertebrae are bones that protect the spinal cord. They can be forced or locked out of their proper positions.(misaligned)
Ligaments and muscles are supportive tissues that can be stretched, torn, or weakened.
Discs are shock absorbers than can bulge, rupture, or wear down.
Nerves can get streched, pinched, or irritated.

Chiropractic Treatment of Low Back Pain in San Diego:

Chiropractic Examination, Testing, and Diagnosis

At Dr Pozhitsky's San Diego Chiropratic Care Center, chiropractic treatment of low back pain begins with a complete chiropractic examination including various tests identifying the cause (diagnosis) of your back pain symptoms.
Note: We will refer you to a medical practitioner if your symptoms are possible indications of a serious medical condition requiring surgery, and patients with these symptoms should seek medical attention immediately. These symptoms include:
  • Severe, continuous abdominal and low back pain (abdominal aortic aneurysm)
  • Sudden bowel and/or bladder incontinence (cauda equina syndrome)
  • Progressive weakness in the legs (cauda equina syndrome)

People with fever and chills, history of cancer with recent weight loss, or who have just suffered a severe trauma should also seek immediate medical attention.

After a diagnosis of low back pain has been establish and the cause is identified, then an individual treatment protocol is designed to correct your back pain problem.

Treatment

Back strain and muscle spasms - About 80% of the patients with low back pain we see at our San Diego chiropractic office have symptoms suggesting an injury to the muscles, ligaments, and vertebrae. The injury is usally a result of over exersion in the case of "weekend warriors" engaged in sports or a lifting injury working around the home, and people suffering from an occupational injury.

For these patients we use the traditional chiropractic adjustment (spinal adjustment) to restore proper function and alignment of the spine. We also use soft tissue and massage therapy to help improve circulation and reduce pain. In addition, we may also use Physical Therapy Modalities such as muscle stimulation, TENS, ultrasound, and heat therapy.

We also commonly prescribe specific strengthening exercises for our patients to prevent low back pain to protect against new or recurring injuries. We also counsel patients on contributing factors to low back pain such as negative lifestyle issues including: Lack of regular exercise • Smoking • Poor diet • Excessive mental stress • Over-reliance on medication • Excessive consumption of alcohol • Poor posture • Improper lifting

D.T.S.-Decompression Traction System
  • Research has proven discs may be a primary site of back and leg pain, as well as neck and arm pain
  • The disc is damaged by "compression & flexion" ['squishing'].
  • The disc is avascular (it has no direct blood supply). It gets its nutrients in and waste products out by diffusion [like a sponge]. The healthier the disc the greater its' diffusion
  • Over time loss of strength of the lower abdominal and low back muscles, poor posture, trauma etc. increase the "compression" on the discs, especially of the lower discs [L4/L5] and the mid-neck discs [C5/C6]
  • DTS therapy stretches the spine in a safe, controlled and very specific way to open the disc spaces. This decreases the pressure on the discs and allows an increase of blood flow to help reduce inflammation and heal the area.
  • Research indicates in some cases bulges may be pulled or drawn in by DTS decompression and help relieve pinched nerves.
  • As with all therapy your adherence to the treatment advise and protocols are vital to ensure the best possible outcome. Additionally, though "Decompression" therapy can help many back conditions it is not a 'cure' for everyone. Many times disc damage cannot be fully healed due to the extent of damage or the continuation of irritating activities.
  • Since traction works with the body's natural healing process it does take time to gain full relief. Twelve sessions is the usual minimum number fir disc injuries, especially with leg or arm radiation [if the condition continues to improve a maintenance program maybe beneficial] & don't skip rehab/exercises
  • "Decompression" is a safe and comfortable treatment for most. It rarely causes pain or worsens symptoms [if traction is painful it is simply discontinued]. Occasionally spasms can create post treatment discomfort (ice or electric stimulation will usually relieve it). Decompression is usually not the treatment of choice for acute pain of less than one week, especially if it is difficult to move and bend.

COMPRESSION/DECOMPRESSION and HERNIATION
Over time, compression weakens the annulus, creating tears, inflammation and possible protrusion of the nucleus. The increase of intradiscal pressure sets the stage for annular compromise, allowing extrusion of the nucleus through the tears (herniation) and possible irritation of the nerve roots.

Negative intradiscal pressure created by "decompressive" traction therapy facilitates nutrition and diffusion into the disc improving healing. In some cases actual "retraction" of the protruded material may take place. However, often an improved healing response reducing inflammation is the source of pain relief.
